Social Services

All people want to feel confident that those they love are safe and happy. This is especially true when someone resides in a nursing home. Family members need to feel secure about placing their loved one in our care. We are very sensitive to this fundamental truth at Dove Healthcare, and we work particularly hard to provide thorough knowledge and resources to each resident and their family.

At Dove Healthcare, we retain a highly qualified and experienced Social Services team. Our social workers take great pride in ensuring that residents and responsible parties are viewed as important elements of the care team. Communication is crucial in short-term and long-term care.

Dove Healthcare Social Workers:

Assist residents and their families with admissions, discharge planning, transfers, and adjustments to living in a nursing home

Strive to identify social and emotional needs of the residents and arrange for their provisions, both internally and externally

Prepare a psycho-social assessment on each resident

Participate in the care planning conference with other treatment staff

Complete on-going educational training to stay up-to-date on rules and laws governing their state certification licenses and to stay abreast on community options and resources

Kris Modl

Kris graduated from the University of Wisconsin - Eau Claire in 1987 where she received her bachelor's degree in social work. She has practiced in long-term care for over 20 years.

Cassie Burrows

Cassie Chrystal, CSW
Admissions Coordinator
At Dove Healthcare Since 2003

Cassie graduated in 2003 from Winona State University (Winona, MN) with a bachelor's degree in social work. Cassie has been practicing for over five years as a nursing home social worker and currently coordinates admissions.

Jenn Gadient

First Floor Social Worker

Jennifer graduated in 2007 from University of Wisconsin - Eau Claire with a bachelor's degree in social work. She has been practicing long-term care social work since graduating. She is pursuing a master's degree in social work through the University of Minnesota.

Jonnalee Pease

Leslie graduated from the University of Wisconsin - Eau Claire with a liberal arts degree in 1991 and a bachelor's degree in social work in 2007. Leslie has experience in both hospital and long-term care social work.

Sherry Kluge

Sherry graduated in 1983 from University of Wisconsin - Eau Claire with a bachelor's degree in social work. She has worked with various populations and has more than 15 years of experience in long-term care.
