Assist residents and their families with admissions, discharge planning, transfers, and adjustments to living in a nursing home |

Strive to identify social and emotional needs of the residents and arrange for their provisions, both internally and externally |

Prepare a psycho-social assessment on each resident

Participate in the care planning conference with other treatment staff |

Complete on-going educational training to stay up-to-date on rules and laws governing their state certification licenses and to stay abreast on community options and resources
