The more you understand about your health and are an active participant in your care, the more you will retain your independence. Licensed staff provides individualized teaching and tools in the areas of diabetic management; medication management; ostomy care; wound care; oxygen therapy; and ambulation and transfer techniques. Support is also available to help with lifestyle changes and information and referral assistance to connect with community resources.

Your care plan will be flexible and tailored to meet your needs.

Your care plan directs our staff on how to care for you by detailing your strengths, weaknesses, preferences, routines, and personal history. The care plan is developed by an interdisciplinary care team through on-going communication with you and your family. It includes activities of daily living, cognition, nutrition, communication, activities, safety, discharge planning, and psycho-social needs.

During your stay at Dove Healthcare - West, your family and you will meet regularly with the care team to review your care. All aspects of your care are discussed at the “care conference” meeting, including your nursing, therapeutic, nutritional, recreational, and spiritual needs. Your discharge plans are also discussed at these meetings.
