Helping seniors manage their healthcare is our mission at Dove Healthcare. Through combined and cooperative efforts, an interdisciplinary team works diligently to promote individualized care that is specific to each resident's needs.
We assist our residents in attaining and maintaining independence. Licensed staff provide individualized teaching and tools in the areas of diabetic management, medication management, ostomy care, wound care, and oxygen therapy. The goal is for the resident to actively participate in their care while at Dove Healthcare and prepare for the process of transitioning home.
Care plans direct our staff on how to care for each resident by detailing their strengths, weaknesses, preferences, routines, and personal history. The care plan is developed by an interdisciplinary care team through on-going communication with the resident and responsible party. It includes activities of daily living, cognition, nutrition, communication, activities, safety, discharge planning, and psycho-social needs.
Each resident and responsible party is asked to participate in planning their care by attending on-going care planning conferences. This is where decisions are made regarding a variety of issues such as nursing care, privacy, medications, and discharge plans.
