Transitions is a progressive, short-term rehabilitation program designed to help residents recover quickly and return home safely. Transitions provides post-hospitalization therapy to restore patients to their normal lifestyles in an atmosphere as close to home as possible. Caring attention is provided by individuals our residents can trust and count on to be there for them. Therapists, nurses, wound specialists, social workers, dieticians, and a physiatrist work together to deliver safe, personalized care in an effective and timely manner.

To assure a safe transition, therapists visit the resident's home and make safety recommendations. With the resident and family present, several living skills are assessed during the visit including cooking, bathing, doing laundry, using stairs, taking medications, and being mobile around the home.
Available to both short-term and long-term residents, restorative therapy combines rehabilitation and nursing skills. It includes range of motion exercises, walking, stretching, and assistance with daily living activities (i.e. getting dressed). This program helps residents retain their level of functioning, restore their independence, and enhance their quality of life. Trained professionals, in coordination with a registered nurse assigned to the resident, deliver the resident's program, provide care, and monitor progression. Oversight is provided by the Nursing Director, and close communication is maintained with the rehabilitation department to ensure optimal goals are reached for each resident.
Nintendo Wii is an interactive gaming system that allows residents to engage mentally and physically in common lifestyle activities such as bowling, tennis, golf, boxing, and baseball. Wii makes it easier for residents to participate in their therapy and adds an element of fun! Balance, coordination, endurance, and fine motor skills are body functions that may improve by using the Wii. A resident that experienced deficits in fine motor coordination as the result of a stroke might add Wii to their therapy regimen because of the obvious involvement of the fingers, hands, and arms in manipulating the gaming system.
At Dove Healthcare, we recruit and hire our own in-house physical, occupational, and speech therapy team. Our therapists live right here in the Chippewa Valley; this loyalty equates to consistency and personalized care for the resident. Each resident has the comfort of knowing the same therapist will follow them through their stay and remain in close communication with the nursing department to ensure optimal care is delivered.

Transportation— We have handicapped accessible vehicles, including a small bus and two vans with wheelchair lifts, for resident transportation to outings, admissions, and medical appointments. Our transportation staff is First Aid and CPR certified.
Pharmacy Services— Our licensed pharmacist reviews each resident's medication regimens and pharmacy procedures. They work with the physician and facility staff to assure each resident receives properly prescribed medications.
Ancillary Medical Services— We offer access to podiatry, dentistry, audiology, and optometry services through an on-site visiting provider. Detailed information about this service provider and their offerings are shared with the resident and family members upon admission.
